The Treatment of Metatarsalgia with Hallux

چکیده
Discussion about the treatment of hallux valgus is largely confined to the merits of various operations on the big toe itself, and the problem of associated metatarsalgia is given rather less detailed consideration. Metatarsalgia and secondary deformities of the lesser toes are common in association with hallux valgus. Bonney and Macnab (1952) recorded metatarsalgia in 45 per cent of the cases reviewed. Rogers and Joplin (1947) found that 75 per cent had dorsal subluxation ofa proximal phalanx and 63 per cent had callosities under the middle three metatarsal heads. When hallux valgus is associated with subluxation of one or more of the lesser metatarsophalangeal joints the patient may have severe metatarsalgia. This can be dealt with by excision of one or more of the painful metatarsal heads, provided that the big toe is stabilised by arthrodesis of the metatarso-phalangeal joint. This paper reports a series of patients in whom this combined procedure was carried out.

Effect of First Ray Insufficiency and Metatarsal Index on Metatarsalgia in Hallux Valgus.
BACKGROUND Two concepts have been proposed to explain the etiology of metatarsalgia in hallux valgus patients: First, as the magnitude of hallux valgus increases, there is a mechanical overload of the lesser metatarsals. Second, increased relative lesser metatarsal length is a factor in the development of metatarsalgia. However, there is no current evidence that these structural factors lead to...
متن کاملComparative study between Wilson and Mitchell metatarsal osteotomies for the treatment of hallux valgus in adults.
Although Mitchell osteotomy and Wilson osteotomy are two popular methods for the treatment of hallux valgus, there are no studies directly comparing their results. Fifty-six patients (73 feet) who underwent a Wilson osteotomy and 30 patients (34 feet) who had a Mitchell osteotomy were followed for a mean period of 33.7 and 38.9 months, respectively. The results were comparable in terms of hallu...
متن کاملLengthening of the shortened first metatarsal after Wilson's osteotomy for hallux valgus.
Metatarsalgia is a recognised complication following iatrogenic shortening of the first metatarsal in the management of hallux valgus. The traditional surgical treatment is by shortening osteotomies of the lesser metatarsals. We describe the results of lengthening of iatrogenic first brachymetatarsia in 16 females. A Scarf-type osteotomy was used in the first four cases and a step-cut of equal ...
متن کاملLoading pattern of postoperative hallux valgus feet with and without transfer metatarsalgia: a case control study
BACKGROUND Postoperative transfer metatarsalgia is a common complication after hallux valgus surgeries. Shortening of the first metatarsal is traditionally thought to be the primary cause of it. However, we speculate the abnormal loading pattern during gait is the real reason. This study is to determine specific differences in the loading patterns between reconstructive hallux valgus (HV) feet ...
متن کاملMetatarsalgia and Lesser Metatarsal Surgery
Several authors have attempted to classify metatarsalgia. Helal classified metatarsalgia into primary, secondary, and unrelated to weight distribution. Primary metatarsalgia is defined as pain across the area of the metatarsophalangeal joints associated with plantar keratoses. It may be a result of functional problems such as wearing a high-heeled shoe or structural problems associated with, fo...
